Abstract
- This article was submitted to JONA but was rejected. We revised and resubmitted to Nursing Management. Identifying the components of an effective transition of care call and key organizational characteristics is necessary to improve outcomes. The purpose of this project was to compare organizations making transition of care calls and those that did not to identify organizational strategies and performance measure differences.
