Diabetes self-management research has historically focused on patient compliance. This study highlighted program and policy factors that influence DSME access and participation. System and program factors need to be more flexible, patient-centered (versus payer-centered), and more effectively aligned with individual readiness and capacity to participate over the patient’s life span. Nursing research should further explore existing program and system factors that hinder DSME participation and advocate for changes toward patient-centered program structure and reimbursement policy that more effectively meet the needs of both patients and providers.
